Licensed Social Worker

Job Summary:
Takes all reasonable steps to assure that social services are provided throughout the Center. Coordinates Social Service activities with other departments. Provides social service support to residents and their families and assists in Admissions processes.
Job Qualifications:
- Bachelor’s Degree in Social Work from an accredited college
- Personal Job-Related Skills: Effective oral and written communication abilities. Counseling skills
Experience:
- One year’s social work experience in health care setting preferred
- Technical Training: None required
Physical and Mental Requirements:
- Duties of the position require moderate standing and walking
- Lifting up to a weight of 25 lbs.
Essential Duties and Responsibilities:
- Provides psycho-social assessments, needs assessments, social histories and participates in interdisciplinary evaluation of individuals' needs for adjustment to placement at the time of their admission
- Develops individual social service care plans designed to meet the needs of the resident, the family and significant others
Other Significant Requirements:
- Maintains confidentiality while working with confidential matters on a daily basis
- Provides service direct to patients while maintaining a positive patient relations atmosphere
Salary: $45,000.00 - $65,000.00/yr (Depends on Experience)
